Dispensation issued May 28, 1898 to form Forsyth Lodge (U.D.). Chartered September 14, 1898 as Hiram Lodge No. 52. Consolidated December 31, 1992, with Hysham No. 129, Hysham, as Hiram No. 52 at Forsyth.
